Job summary

Visiting Nurse and Hospice for VT & NH (VNH) is seeking a Licensed Practical Nurse to provide home health care services in White River Junction, VT and surrounding communities. You will support the RN in implementing the plan of care and ensure patient comfort and safety in the home setting.

The role emphasizes autonomy, critical thinking, and compassionate patient education, with opportunities to collaborate in case conferences and contribute to high-quality home-based care.

Qualifications

• Active LPN license in VT and NH.
• 1–2 years’ home care experience preferred.
• Clear spoken and written communication skills.
• Ability to adapt to changing schedules.

Responsibilities

• Provide prescribed patient care delegated by the RN.
• Participate in patient case conferences as appropriate.
• Assist the RN with plan of care evaluation and modification.
• Observe and report patient needs and reactions to treatment.
• Provide nursing care with privacy, comfort and safety in mind.
• Instruct patients in home and community programs.
• Administer medications after successful competency.
• Document patient information clearly and timely.
