UVM Home Health & Hospice in Vermont is seeking a compassionate LPN to join our Adult Home Health team. You will provide skilled nursing care in patients' homes under the supervision of an RN.

This role emphasizes assessment, education, and collaboration with a multidisciplinary team to deliver high-quality, patient-centered care.

Qualifications

• Active LPN license in Vermont.
• Two or more years of related home health, hospice, and/or maternal/child health nursing experience.
• Strong assessment, communication, and interpersonal skills.

What You Bring
