Job summary

Billet Health is seeking Licensed Practical Nurses (LPNs) for Part-Time / Per Diem roles in West Valley & Surrounding Areas. The team provides home health care to geriatric patients, with shifts that fit your schedule. Compensation is $33-45 per hour DOE, and travel within your established territory is included.

Qualifications

• Active LPN license in the state of practice.
• At least 1 year of Home Health experience.
• Valid Driver's License.
• Valid Auto Insurance.
• CPR or BLS Certification.
• AZ Level One Fingerprint Clearance Card (Arizona applicants only).
• Physical/medical clearance to perform job duties (Nevada applicants only).
• Valid TB test in last year or chest X-ray in last five years.

Responsibilities

• Provide skilled nursing care to patients in homes or residential facilities.
• Assess patient needs and communicate changes to the RN Case Manager and Director of Nursing.
• Follow plans of care and provide nursing interventions.
• Monitor and document patient progress and care provided.
• Travel to patient locations within assigned service area.

Billet Health is looking for Licensed Practical Nurses (LPNs) who bring both clinical expertise and a genuine heart for patient care. Our Home Health team provides compassionate, individualized care to geriatric patients in the comfort of their homes or residential facilities throughout our service areas.

As an LPN with Billet Health, you will work alongside an RN Case Manager and Director of Nursing to assess patient needs, provide quality care, and support each patient throughout their 9-week episode of care.

As patients come onto service, our Staffing Team will contact you when coverage is needed. You choose how much you want to work. Whether you want to see several patients a day or just a few visits each week, you have the flexibility to set your own pace.

• Flexible scheduling – You coordinate visit times directly with your patients.
• Choose your workload – Take on as many or as few visits as fit your schedule.
• No assigned visit times – You arrange visits based on what works best for you and your patients.
• Typical visits are 45–60 minutes.
• Travel included within your established territory.
• Additional compensation may be available for patients outside your normal territory on a case-by‑case basis.
• Work independently while having the support of our clinical and staffing teams.
• Gain experience in Home Health with support from an experienced team.

What You'll Do

• Provide skilled nursing care to patients in their homes or residential facilities.
• Assess patient needs and communicate changes in condition to the RN Case Manager and Director of Nursing.
• Follow established plans of care and provide appropriate nursing interventions.
• Monitor and document patient progress and care provided.
• Build meaningful relationships with patients and their families.
• Deliver both excellent clinical care and a compassionate patient experience.
• Travel to patient locations throughout your assigned service area.
