Job summary

TheKey in Vienna, VA is seeking an LPN Supervisor to lead clinical oversight for home-based care clients across Vienna. You will conduct initial intakes, in-home assessments, update care plans, supervise caregivers, and ensure regulatory compliance under the Director of Nursing.

This role requires an active Virginia LPN license, 3–5 years in home health, strong teaching skills, and reliable driving for local travel up to 75%. Compensation includes hourly pay and full benefits.

Qualifications

• Current, active Virginia LPN license.
• 3–5 years of nursing in home care, elder care, or related clinical setting.
• Experience teaching, coaching, and evaluating caregivers in home or classroom settings.
• Strong customer service, listening, and conflict-resolution skills.
• Proficiency with electronic records/intake software and documentation tools.
• Valid driver’s license and reliable transportation for local travel 50–75% within Virginia.

Responsibilities

• Assessments & client onboarding: nursing assessments in client homes and individualized care plans.
• Supervisory visits & care oversight: regular in-home visits and QA assessments, including high-acuity clients.
• Caregiver training & mentorship: lead hands-on training and retraining for caregivers.
• Incident management & QA: address incidents, retraining, and plan updates.
• Care coordination: collaborate with scheduling/staffing for best caregiver-to-client match.
• Documentation & compliance: maintain client records and shift notes per state licensure.
• Community engagement: support local branding and outreach with Home Care Liaisons.
