Job summary

Trilogy Health Services, LLC is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct clinical care for residents and supervise nursing assistants on the unit. You will observe and respond to residents’ physical and emotional changes, document care, and collaborate with the charge nurse, physicians, and leadership.

The role requires maintaining LPN licensure and CPR certification, with a focus on holistic, patient-centered care in a long-term care setting.

Qualifications

• Must hold current, valid LPN license.
• CPR certification required.

Responsibilities

• Visit residents daily to assess physical and emotional status and notify care team of changes.
• Identify problems and initiate lifesaving measures in absence of a physician.
• Provide holistic care within LPN scope.
• Monitor medications, supplies, and equipment to ensure adequate stock.
• Complete medical forms, reports, and charting as needed.
• Report occupational exposures per facility policies.
