Job summary

Artis Senior Living of Branford is seeking a Coordinator of Health & Wellness (LPN/LVN) for a full-time, 2nd shift (3-11pm) role in Branford, CT. The position focuses on clinical monitoring of residents with Alzheimer's/Dementia, medication administration, care planning, and supervising Caregivers and CNAs.

Starting pay is $34/hour. The role supports residents’ daily needs with safety and dignity, and offers a collaborative team environment with opportunities for growth and professional

Qualifications

• Must possess current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation.
• Experience in rehabilitative or geriatric nursing preferred.
• Licensure must be in good standing with the State Board of Nursing.

Responsibilities

• Take an active role in resident assessments and development of service plans.
• Oversee and implement the self-administered medication management program.
• Administer medication and treatments per physician orders.
• Document and report care provided and resident observations.
• Provide emotional support and ensure safety of residents.
• Supervise Caregivers and CNAs in resident care.
• Assist with marketing and outreach to prospective residents.
