Lakeside Generations, a Cassia senior community, is hiring Licensed Practical Nurses (LPNs) to join our team as a Health Unit Coordinator (HUC) to assist and support the Director of Nursing. This is a full-time, benefits-eligible position with on-call rotation, located in Dassel, MN.

As an LPN HUC, you will coordinate information, support administrative tasks, and ensure an excellent resident experience. Prior HUC or TMA experience and Minnesota licensure are required; wage range is $30–$34.82

Qualifications

• Must have Minnesota LPN or RN license.
• Experience as Health Information Coordinator, HUC or TMA required.
• Must pass medical terminology class.
• Proficient with computers, word processing, data processing and copiers.
• Strong leadership and communication skills (written and verbal).
• Excellent organizational and time-management skills; team-building ability.
• Self-starter and team player.

Responsibilities

• Displays courtesy and respect for residents, families, and staff.
• Acts as bridge between Centralized Admissions and site clinical team for smooth transfers.
• Maintains positive relationships with Centralized Admissions Team and referral sources.
• Coordinates equipment needs for patient stays.
• Documents, maintains paperwork, records and reports.
• Enters physician orders with high accuracy.
• Supports scheduling medical appointments and transportation.
• Maintains electronic medical records for census data.
• Assists with inventory and delivery of nursing supplies and medications.
• Balances HUC duties with LPN duties as needed.
• Performs other duties as assigned.

As an LPN Health Unit Coordinator (HUC) of the job, you will coordinate information and provide administrative support to our team to ensure our residents and visitors have the best possible experience. Our ideal candidate has a knowledge of medical terminology and prior experience as a Health Unit Coordinator.

Position Type: Full-Time, benefits eligible position. Position includes on-call rotation.

Wage Range: $30 - $34.82 / hour depending on experience.

Health Unit Coordinator (HUC) responsibilities:

• Displays a courteous attitude and respect for all residents, families, and staff
• Acts as the bridge between Centralized Admissions Team and site clinical team ensuring a smooth transfer of patients
• Maintains positive relationships with Centralized Admissions Team, supporting as liaison for site
• Partners with Centralized Admissions Team on maintaining positive relationships with current referral sources
• Anticipates, coordinates, and orders any equipment needed for a patients stay
• Documents, maintains paperwork, records and reports
• Enters physician's order with high degree of accuracy
• Supports scheduling medical appointments and transportation
• Maintains electronic medical records for accurate census data
• Assists with inventory and delivery of nursing supplies including assistance in managing medications, supplies, and equipment including ordering, receiving, storing and disposing of all items in accordance with policies, and procedures
• Balances Health Unit Coordinator duties with LPN duties and needs from clinical team
• Performs other duties as assigned

Health Unit Coordinator (HUC) Qualifications:

• Current Minnesota LPN or RN Required
• Prior experience as a Health Information Coordinator, Health Unit Coordinator (HUC) or Trained Medication Aide (TMA) required.
• Must be able to pass medical terminology class
• Basic knowledge of computers, word processing, data processing, and copy machines
• Strong leadership, written and verbal communication skills
• Excellent organizational, time management and team-building skills
• Self-starter with a desire to be a part of a team
