Job summary

SageGrove is seeking a Health Services Coordinator (LPN) in Olathe, KS to improve resident well-being and coordinate care. You’ll conduct assessments, support regulatory readiness, and build strong relationships with residents, families, and partners.

The role emphasizes hands-on care, training caregivers, and ensuring compliant care plans and medication records. A valid LPN license and CPR/BLS are required.

Qualifications

• Active state nursing license and CPR/BLS certification.
• 1+ year supervisory or leadership experience in a senior living or related care setting.
• 1+ year experience providing care to cognitive/memory-care residents (preferred).
• Valid driver’s license and acceptable driving record; ability to transport residents.
• Ability to assist residents with limited mobility and lift up to 50 lbs.
• Strong verbal and written communication; ability to collaborate and make decisions.
• Proficient with Word, Excel, PowerPoint and HIPAA privacy.

Responsibilities

• Conduct comprehensive resident assessments prior to move-in and ongoing.
• Assist with community readiness for regulatory inspections; maintain service plans and med records.
• Establish and foster relationships with residents, families, associates, home health, hospice, and external partners.
• Coordinate health-related training for caregivers and conduct skill observations.
• Provide direct resident assistance including personal care and medication administration as ordered by physicians.
