Job summary

Homestead of Olathe Memory Care is seeking an LPN to join our team in Kansas. The role involves providing care to patients per care plans, communicating with patients and families about education and progress, and maintaining clinical records in a compliant environment.

Qualifications

• Provide care to patients per their care plans and may confer with physicians, patients, families and other staff.
• Communicate with patient and family regarding education and progress of patient.
• Review and maintain all clinical records required for patient care and progress.
• Review weekly reports to monitor staff compliance with Kansas and Federal requirements including blood borne pathogens, hazardous materials, infection control, and fire safety.

Responsibilities

• Provide a provision of care to patients in accordance with their care plans.
• May require conferences with physician, patient, family members and other staff involved with patient’s care.
• Communicate with patient and family regarding education and progress of patient.
• Review and maintain all clinical records required for patient’s care and progress.
