SageGrove in Lenexa, KS is seeking a Health Services Coordinator (LPN) to lead clinical care for residents and coordinate health services. You will conduct assessments, ensure regulatory readiness, and collaborate with families, staff, and partners to deliver high-quality care.

Ideal candidates hold an active state nursing license, CPR/BLS certification, and at least 1 year of supervisory experience in a senior living setting, with strong communication and HIPAA practices.

Qualifications

Active Nursing license in the state.
Current CPR/BLS certification.
1+ years in a supervisory or leadership role in a senior living or related industry.

Responsibilities

Conduct comprehensive resident assessments before move-in and ongoing.
Assist with regulatory readiness including service plans and records.
Foster relationships with residents, families, and external partners.
Coordinate health-related training for caregivers and observe skill delivery.
Provide direct resident assistance and administer medications per physician orders.
