CHESCO Services in Chesterfield, SC is seeking an LPN-Health Coordinator for a full-time role (Mon-Fri) with weekend on-call rotation. You will provide nursing care under the supervision of the Healthcare Supervisor/Director of Nursing and coordinate services with community health professionals.

Responsibilities include routine nursing procedures, care plan documentation, medication coordination, and maintenance of medical charts. Some on-call and travel within the agency may be required.

Qualifications

  • High school diploma and completion of approved nursing program.
  • State certification by SC Board of Nursing.
  • Maintain certification in First Aid, CPR, and CPI.
  • Valid SC driver's license and ability to operate a 15-passenger or smaller vehicle.
  • Furnish proof of liability insurance.

Responsibilities

  • Provide general nursing care to CHESCO Services clients under supervision.
  • Perform routine nursing procedures and maintain medical documentation per physician orders.
  • Coordinate with outside health agencies and act as liaison with health professionals.
  • Assist in admission evaluations and develop health-related training plans.
  • Schedule and assist with medical reviews and ensure medications are administered as prescribed.
  • Maintain and update medical charts and provide nursing summaries.
  • Provide 24/5 on-call coverage and coordinate coverage within 45 minutes of response range.
  • Schedule medical appointments and ensure communication of medical status to Coordinators.
  • Contribute to staff and caregiver in-services on health issues.
  • Participate in interdisciplinary team meetings and prepare nursing reports.

HEALTHCARE COORDINATOR/LPN – Pee Dee (Pageland) Divison


REPORTS TO: Healthcare Supervisor/DON


WORK HOURS: 40 Hour Work Week


SUPERVISES: None


QUALIFICATIONS: High school diploma and completion of approved school of nursing, with state certification by S.C. Board of Nursing. Acquire and maintain certification in First Aid, CPR, and CPI (non-crisis intervention). Subject to annual TB screening. Valid driver’s license from state of residence and ability to operate motor vehicle for 15 passengers or less. Furnish proof of liability insurance.


JOB SUMMARY: Provides nursing care for CHESCO Services Clients under supervision of the Healthcare Supervisor/Director of Nursing.


PHYSICAL Climbing, balancing, stooping, kneeling, crouching, crawling, reaching, ABILITIES standing, walking, pushing, pulling, lifting, (patients with assistance), INCLUDE: fingering, grasping, feeling, talking, hearing, repetitive motions, communication, writing, reading, driving (including nights), counting, work double shifts as necessary, running, and smelling.


PHYSICAL REQUIREMENTS INCLUDE: Very heavy lifting with assistance, visual acuity must be accurate to write and read small print and record data. Worker is subject to both indoor and outdoor environments and maybe subject to temperature extremes of heat and cold for short time periods. The worker is subject to noise and hazards including: the possibility of physical assault, property destruction, and infections. The worker is subject to wear protective clothing to prevent exposure to infection.


JOB DUTIES


  1. Provides general nursing care to CHESCO Services individuals as per local, state and federal guidelines.

  2. Provides routine nursing procedures (medical appointments, vital signs, etc.) and paperwork (care plan, MAR, progress notes, TD assessments etc.) per agency medical guidelines and in accordance with M.D.’s orders.

  3. Responsible for coordination of services with outside health agencies. Serves as primary Liaison for all community health professionals and promotes effective working relationships with these professionals.

  4. Participates in the pre-admission evaluation of individuals selected for placement in terms of nursing services. Assists in the development of training programs upon admission within the health related areas. Complete necessary appointments and paperwork for new admissions.

  5. Schedules, attends and assists Coordinators with the Drug Reviews. Ensures Individuals receive appropriate medications as prescribed by the psychiatrist. Initiate consent forms for psychotropic medications.

  6. Monitors and notes monthly any discrepancies in the areas of medication administration records, medication storage, medication supplies (prescribed and OTC meds), follow up on MD appointments, implementation of MD recommendations, vital signs, and dietary needs. Provides monthly nursing summaries to the Coordinators.

  7. Provides 24 hour/5 days a week and shared week-end and holiday Aon call@ medical coverage for individuals. If not within a 45 minute response range, prior notification must be given to Supervisor.

  8. Schedules all medical appointments for individuals. Coordinates with Coordinator or Caregiver to provide coverage to the appointments as needed.

  9. Ensures Coordinators are updated on the medical status of the individuals.

  10. Responsible for the maintenance and upkeep of the medical charts; ensures that the medical charts are current.

  11. Provides and documents staff/consumer/caregiver in-services/training relating to medical/health issues.

  12. Provides health care counseling to individuals as it relates to his or her personal needs; smoking, diet, exercise, proper hygiene, etc.

  13. Participates as a member of the interdisciplinary team in single plan meetings, medication reviews, etc. and prepares required nursing reports.

  14. Completes other duties as identified or assigned.
