Job summary

Folkston Park Care and Rehab is seeking a Licensed Practical Nurse (LPN) to support short-term rehabilitation and long-term care needs. The LPN will collaborate with the Unit Director to develop and implement individualized nursing care plans, ensuring patient needs are met.

The role includes assisting in orientation and supervision of nursing staff, managing daily unit operations per shift, and assuming leadership responsibilities as required.

Qualifications

Licensed Practical Nurse (LPN) must be a graduate of an approved school for Practical or Vocational Nursing.
Currently licensed by the Georgia State Board of Nursing.

Responsibilities

Assist in developing and implementing individualized Nursing Care Plans.
Support unit operations per shift and supervise nursing personnel as directed.
Take on leadership responsibilities as needed and coordinate with the Unit Director.
