The Terraces of Phoenix is a HumanGood community seeking Licensed Practical Nurses (LPN/LVN) to join our Skilled Nursing team. The role focuses on person-centered care, safety, and resident satisfaction in a vibrant senior living environment.

We offer full-time, part-time, and PRN positions with multiple shifts. Competitive pay ($30-$36/hour DOE) and comprehensive benefits support your health and financial security.

Qualifications

• Current LPN/LVN license in the state of practice.
• CPR certification current.
• Minimum 6 months experience in Skilled Nursing.
• Experience in post-acute, sub-acute, or senior care nursing preferred.
• PointClickCare software experience.

We are currently seeking a Licensed Practical Nurses (LPN/LVN) to join our Skilled Nursing team and make a meaningful difference in residents’ daily lives.

Pay Rate: $30-$36/hour (DOE)

Positions Available

• Full-time, 6 AM - 2 PM and 2 PM - 10 PM: Week 1: Mon., Tues., Fri., Sat; Week 2: Sun., Wed., Thurs., Fri.
• Part-time, 2 PM - 10 PM: Week 1: Mon., Fri., Sat; Week 2: Sun., Thurs., Fri.
• PRN: Must be available all shifts, including weekends and holidays

In this role, you will:

• Provide attentive nursing care that reflects our Philosophy of Person-Directed Care
• Help create a safe, supportive, and engaging environment for residents, staff, and visitors
• Coordinate assignments with care partners to ensure excellent resident care
• Support and hold team members accountable for quality care and resident satisfaction
• Monitor resident needs and respond with compassion and professionalism

To thrive in these roles, you should have:

• Current LPN/LVN license in the state of practice
• PointClickCare (PCC) software experience
• Minimum 6 Mos. experience in Skilled Nursing (current or recent)
• Current/prior experience in post-acute, sub-acute, or senior care nursing preferred
• Current CPR certification
