Job summary

Health Partners of Western Ohio is seeking a Licensed Practical Nurse (LPN) to join our team at the Tiffin Community Health Center. This full-time position supports providers with hands-on clinical tasks, patient education and collaboration within a patient-centered medical home.

The ideal candidate has a nursing degree and Ohio LPN license, CPR-BLS certification, and 1–3 years of nursing experience. Flexible hours and ongoing opportunities are offered in a diverse, community-focused setting.

Qualifications

• Graduate of a school of nursing accredited by National League of Nursing.
• Current license to practice as an LPN in the State of Ohio.
• CPR – BLS (basic life support) required.
• One to three years full-time experience in nursing preferred.
• Ability to work as a team member and with a culturally diverse group.

Responsibilities

• Collects patient data through physical examination and observation.
• Documents nursing care per policy.
• Formulates plan of care under provider direction.
• Administers medications as prescribed.
• Follow universal precautions and approved protocols.
• Participates in quality improvement activities and education.
