Job summary

Woodcrest of Country Manor in St. Joseph, MN, seeks an LPN for full-time day shifts in assisted living. You will work with a coordinated nursing team under the RN Case Manager and Executive Director of Clinical Assisted Living to deliver high-quality care.

The role emphasizes care planning, medication administration, resident assessments, and compassionate patient interactions in a vibrant senior living community.

Qualifications

• Current license in good standing as an LPN in Minnesota.
• CPR certification required.
• Previous nursing or medical field experience preferred.
• Ability to professionally represent Woodcrest's Assisted Living Department.
• Familiarity with medical tools and equipment.
• Ability to build rapport with residents.
• Compassionate, caring demeanor.
• Excellent written and verbal communication skills.

Responsibilities

• Assist in developing resident care plans and review care plans as requested.
• Provide nursing services per established care plans and document changes.
• Visit residents to assess status and implement nursing interventions.
• Assist residents to maintain self-care and independence.
• Administer medications and maintain treatment records.
• Communicate relevant information to supervising RN, physicians, and family.
• Complete admissions; weekly, follow-up, and discharge charting.
• Provide support and supervision to nursing staff as needed.
• Maintain positive communication with patients and families.
• Perform other duties as assigned.
