Job summary

Collage Rehabilitation Partners in Covington, LA is seeking a Full-Time LPN to support our Residential Program. The role requires nursing licensure and involves coordinating care for adults with brain injuries in a structured setting.

The LPN will assess residents, implement evidence-based care plans, train clients and families, and supervise CNAs, with options for on-call rotation. A valid driver's license is required for community visits.

Qualifications

• Licensed LPN required.
• 1-2 years nursing experience preferred.
• Valid driver's license and responsible driving behaviors required.
• Ability to lift 50 lbs.

Responsibilities

• Provides nursing and facilitation services for assigned clients within a designated site and is responsible for assessment and interventions for assigned residential clients.
• Designs and implements treatment strategies based on nursing theory, evidence based practice and principles to promote health and medical stability.
• Provides training to clients or family members regarding how to promote health, wellness and medical stability as well as to staff.
• Directs CNAs on the unit and may participate in the program’s on-call rotation.

We are currently seeking a Full-Time LPN to support the needs in our Residential Program in Covington, LA.

Schedule: 7a-3p or 11p-7a

• Provides nursing and facilitation services for assigned clients within a designated site and is responsible for assessment and interventions for assigned residential clients
• Designs and implements treatment strategies based on scientific nursing theory, evidence based practice and principles to promote health and medical stability
• Provides training to clients or family members regarding how to promote health, wellness and medical stability as well as to staff
• Directs CNA's on the unit. May participate in program’s on-call rotation.

Requirements

• Licensure as an LPN is required
• 1-2 years’ experience in nursing is preferred
• Must have a valid driver's license and responsible driving behaviors must be maintained
• Ability to lift 50lbs
