ReMed, a Collage Rehabilitation Partner, invites qualified LPNs to join its Residential Program in Covington, LA. The role provides nursing and facilitation services for assigned clients, focusing on assessment, interventions, and health stability within a supportive team environment.

The position requires a valid LPN license, 1–2 years of nursing experience preferred, and the ability to lift 50 lbs. A driver’s license is required, and on-call rotation may be involved.

Responsibilities

• Provides nursing and facilitation services for assigned clients within a designated site and is responsible for assessment and interventions for assigned residential clients.
• Designs and implements treatment strategies based on nursing theory and evidence-based practice to promote health and medical stability.
• Provides training to clients or family members regarding health promotion and medical stability, as well as to staff.
• Directs CNAs on the unit and may participate in on-call rotation.
