Job summary

Distinctive Living in Maumee is seeking a dedicated LPN/LVN (Full-Time) to assess resident health status and develop care plans under the Director, Health and Wellness. The role emphasizes compliance with state regulations, medication administration oversight, and staff training.

The position requires licensure and nursing experience in assisted living or memory care, with opportunities for professional growth and a supportive team environment.

Qualifications

• High School Diploma or equivalent required.
• LPN/LVN licensure required.
• Minimum one year of nursing or caregiver experience preferred in assisted living, home health or memory care.
• Minimum six months supervisory experience.

Responsibilities

• Follows the services outlined in each resident's service plan and documents changes.
• Communicates with the Director, Health and Wellness as needed to ensure quality services.
• Provides hands-on care and nursing tasks aligned to the resident's care plan and state regulations.
• Trains care staff and supports continuing education requirements.
• Transcribes physicians' orders and reviews new orders.
• Maintains required certifications and participates in in-service trainings.
• Supports CQI program and reports issues to Executive Director.
• Reviews MARs and Daily Reports to ensure proper interventions.
• Follows HIPAA, OSHA, and Blood Borne Pathogen guidelines.
• Promotes teamwork and professionalism; performs other duties as assigned.
