Job summary

D&S Community Services, part of Sevita, is seeking an LVN/LPN licensed nurse for San Angelo, TX. The role focuses on health monitoring, medication administration, and coordinated care under physician orders.

Qualifications

• Associate’s degree in Nursing or related field, or state certificate.
• One year of nursing experience with the target population.
• Current LPN/LVN licensure in the state of residence.
• Valid driver’s license, car registration and auto insurance.
• CPR/FA and HIV/AIDS, BBP, ACT trainings or equivalent certifications.
• Ability to effectively communicate with staff, individuals served, and families.

Responsibilities

• Monitor the health conditions of individuals served and ensure quality services are provided.
• Administer medications and document, supervise and monitor staff who administer medications.
• Provide prescribed medical treatment by physician’s orders and personal care services.
• Conduct assessments and plan health care services for each ISP, respond to changes.
• Provide training to staff on universal precautions, infection control, and medication administration.
• Liaise with medical professionals and external agencies to coordinate services.
• Provide training on self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
