Job summary

Berlin Rehabilitation and Healthcare Center, a 128-bed sub-acute and long-term care facility, is seeking a Licensed Practical Nurse (LPN) to join our compassionate team.

We offer full-time, part-time, and per-diem positions with flexible shifts (7PM-7AM; 7AM-7PM; 7AM-3PM; 3PM-11PM) and same-day pay. The role requires NJ LPN license, BLS, and prior LTC/PCC experience; compensation ranges from $30 to $35 per hour, all-inclusive.

Qualifications

• Active, unencumbered LPN license in New Jersey.
• BLS Certification (CPR) required.
• Experience in Long-Term Care or Subacute settings preferred.
• Familiarity with PCC software is a plus.

Responsibilities

• Provide patient care according to plan of care and physician orders.
• Follow individualized care plans developed by the Nurse Manager and team.
• Administer medications and treatments per care plan and policies.
• Monitor resident conditions and report changes promptly.
• Maintain accurate nursing documentation and ensure a safe environment.
