Job summary

Berlin Rehabilitation and Healthcare Center is seeking a Licensed Practical Nurse (LPN) for full-time, part-time or per-diem roles. Shift options include 7PM-7AM (temp Aug–Nov) and other day/evening blocks with a competitive $30.00-$35.00 per hour, all inclusive.

Qualifications

• Current, unencumbered active LPN license in New Jersey.
• Valid BLS Certification (CPR) required.
• Prior experience in Long-Term Care or sub-acute settings.
• Prior experience with PointClickCare (PCC).
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice and written plan of care.
• Follow and implement individualized care plans developed by Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to supervising nurse or physician.
• Maintain accurate, timely documentation per physician orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
