Job summary

Grace Lutheran Communities in Altoona, WI is seeking a dedicated Licensed Practical Nurse (LPN) to join the River Pines team. We offer Full-Time and Part-Time AM/PM/NOC shifts with new wages starting at 28.00 to 33.50 per hour, depending on experience.

As an LPN on our care team, you will perform and document nursing skills per care plans, document medications and treatments, provide direct resident care, and collaborate with RNs to monitor residents' health and respond to changes.

Qualifications

• Valid LPN license
• Ability to perform basic nursing skills per residents' care plans
• Ability to document care observations accurately

Responsibilities

• Perform and document basic nursing skills within licensure and care plans
• Document care observations, medications, and treatments promptly and accurately
• Provide direct resident care as needed
• Transcribe physician orders
• Monitor resident health status under RN direction
• Replace absent staff when necessary
