Job summary

PureHealth is seeking an LPN for a full-time role with day and night 12-hour shifts in a post-acute setting. You will render nursing care under RN direction, document in EMR, administer medications, and participate in care plan development with the health team.

You will collaborate across departments, ensure safety, and contribute to patient education and discharge planning. CPR and state license are required; benefits include tuition reimbursement and flexible scheduling.

Qualifications

• Graduate of an accredited LPN program and valid license are required.
• Basic nursing duties and patient care experience preferred.
• CPR certification is required.

Responsibilities

• Provide nursing care per the plan in assigned area.
• Initiate and lead team conferences for care plans.
• Administer medications accurately and monitor patient responses.
• Document observations and interventions in EMR.
• Collaborate with the care team to adjust plans as needed.
