Job summary

PureHealth is seeking a licensed practical nurse (LPN) to provide nursing care in a post-acute setting. The LPN reports to the Unit Manager and collaborates with the Registered Nurse to meet patient needs.

Responsibilities include administering medications, implementing care plans, and documenting in the EMR while ensuring infection control and patient safety. Weekend and holiday shifts are required in a 24/7 environment.

Qualifications

• High School Diploma or Equivalent required.
• Graduate of an accredited LPN program with licensing.
• Current CPR certification and valid LPN license.

Responsibilities

• Provide safe, individualized nursing care per plan.
• Develop and update care plans with the Unit Manager.
• Administer medications accurately and monitor responses.
• Document care in the EMR and communicate with team.
