Qualifications

• Must hold and maintain a current LPN license.
• CPR certification required; recert within 90 days of hire.
• License in good standing with no abuse, neglect, or misappropriation findings.

Responsibilities

• Provide quality nursing care using the nursing process.
• Identify needs and input for discharge planning
• Develop individualized care plan with resident and care team
• Implement care plan focusing on resident-centered outcomes with available resources
• Collaborate with physician and other health care professionals; share changes to care plan
• Deliver and maintain optimum resident care and comfort.
