Job summary

Brickyard Healthcare is seeking a Licensed Practical Nurse (LPN) to join our care team in Richmond, Indiana. The successful candidate will provide quality nursing care, participate in discharge planning, and coordinate with an interdisciplinary team to ensure resident-centered outcomes.

Qualifications

• Must hold and maintain a current LPN license.
• Must hold and maintain CPR certification within 90 days of hire.

Responsibilities

• Deliver quality nursing care using the nursing process with a focus on outcomes.
• Identify resident needs and contribute to discharge planning and follow-up during stay.
• Collaborate with the care team to develop an individualized plan of care with residents and families.
• Implement and monitor the plan of care cost-effectively while maintaining resident comfort.
• Share information with physicians and other health care professionals for plan updates.
• Deliver and maintain optimal resident care by applying current nursing practices.

Responsibilities:

• Assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process.
• Identify needs and provide input for discharge planning and follow-up throughout length of stay
• Assist with development of individualized plan of care in collaboration with the resident/responsible party and interdisciplinary care team
• Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes by utilizing all appropriate, available resources for resident, family or responsible party counseling and discharge planning.
• Work in collaboration with physician and other health care professionals by sharing information relevant to changing plan of care
• Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practice

Qualifications:

• Must hold and maintain a current license to practice as a Licensed Practical Nurse (LPN)
• Must hold and maintain a current CPR certification. Licensed nursing staff are required to obtain their CPR certification within 90 days of hire.
• License in good standing with the state. Must have no finding in the registry concerning abuse, neglect, or misappropriation of resident property.
