Healthy Community Health Center in Harrisonburg, VA is seeking a Licensed Practical Nurse to join our patient-centered care team. The LPN will support providers, coordinate patient care, and help maintain quality and safety in a busy clinical setting.

This role emphasizes routine immunizations, medications, data entry into EMR, vital signs, and adherence to HIPAA and NCQA Patient-Centered Medical Home standards. Travel to other HCHC locations may be required.

Formación

• Must demonstrate good internal and external customer service.
• Ability to support patient care within the NCQA Patient-Centered Medical Home model.
• Will obtain ongoing education consistent with level and area of practice.
• Ability to react to changes in duties and patient volume.

Responsabilidades

• Contributes to and participates in a healthy clinic team culture.
• Performs focused patient assessments and data collection.
• Administers routine immunizations, injections, medications and skin tests.
• Documents nursing procedures and EMR data accurately.
• Performs vital signs, demography data and counseling.
• Assists with emergencies and demonstrates leadership during events.
