Health Partners of Western Ohio is seeking an LPN to join our Defiance/Bryan Community Health Center team. You will support providers, perform hands-on clinical tasks, educate patients, and collaborate with a diverse team to deliver high-quality, patient-centered care.

This role requires an Ohio LPN license, CPR/BLS certification, and flexible hours to meet patient needs. We offer a supportive environment, opportunities for growth, and mileage reimbursement for travel between locations.

Qualifications

• Graduate of a school of nursing accredited by NLN.
• Current license to practice as an LPN in Ohio.

Responsibilities

• Collects patient data through physical examination, interview and observation.
• Documents nursing care according to policy.
• Formulates plan of care under provider direction.
• Administers medications as prescribed.
• Assists with patient education and care coordination.
