Job summary

Health Partners of Western Ohio is seeking a Licensed Practical Nurse to join a patient-centered team at Defiance/Bryan Community Health Center. You will support providers with clinical tasks, patient education, and teamwork in a fast-paced setting.

Responsibilities include data collection, medication administration, following care plans, and maintaining supplies. A current Ohio LPN license and CPR-BLS are required; travel between locations may be necessary.

Qualifications

• Graduate of a school of nursing accredited by National League for Nursing.
• Current license to practice as an LPN in the State of Ohio.
• CPR–BLS certification required.

Responsibilities

• Collect patient data through examination, interview and observation.
• Document nursing care per policy and procedure.
• Formulate care plans under provider direction.
• Administer medications as prescribed by the provider.
• Follow approved protocols and universal precautions.
• Participate in Quality Improvement activities.
• Educate patients and provide discharge instructions.
• Maintain stocks of medicines and medical supplies.
• Assist with seasonal programs and teamwork.
