Discovery Village at the Forum in Fort Myers, FL is seeking a Licensed Practical Nurse (LPN) to provide compassionate, high-quality nursing care while supporting resident safety and well-being.

You will work part-time, with a pay rate of $30-$32 per hour, performing assessments, medications administration, care planning, and collaborating with the interdisciplinary team to deliver person-centered care.

Qualifications

• Current LPN license required in the state.
• Minimum 2 years of Licensed Nurse experience.
• Experience in assisted living/long-term care preferred.
• Strong clinical judgment, problem-solving, and decision-making skills.
• Ability to work nights, weekends, and flexible schedules.
• Proficiency with electronic documentation systems.

Responsibilities

• Provide hands-on nursing care per resident care plans.
• Monitor residents' physical and psychosocial status and changes.
• Administer medications and treatments as prescribed.
• Coordinate and communicate resident care needs with physicians, families, and pharmacies.
• Support health promotion and wellness initiatives within the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning and resident evaluations.
• Collaborate with caregivers and leadership to ensure high-quality care.
