Job summary

Premise Health Inc. is hiring a Full Time Licensed Practical Nurse for our Health Center in Hamilton, OH. You will provide direct patient care to employees and dependents, with clinic hours following a 30-40 hour week schedule.

Responsibilities include medication administration, wound care, specimen collection, and maintaining patient records. Requires an active LPN/LVN license, current BLS, and at least three years in ambulatory or related settings.

Qualifications

• Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school.
• Currently Licensed Practical Nurse in state of practice.
• Current Basic Life Support certification (AHA/ARC or equivalent).
• At least three years of clinical experience as LPN/LVN in ambulatory, occupational health, or ED setting.

Responsibilities

• Provides direct patient care under supervision of healthcare providers within scope of practice.
• Documents and administers medications, wound care, and collects specimens per provider orders.
• Maintains sterile technique andFollow infection control procedures; keeps equipment clean and functional.
• Maintains accurate patient health records and documentation.
• Performs additional duties as assigned by supervising healthcare providers.
