Job summary

Foundation Medical Partners Inc is hiring a Licensed Practical Nurse (LPN) for Pepperell Family Practice. You will deliver patient-centered nursing care under supervision, participate in care coordination, and educate patients in self-management and wellness.

Responsibilities include assessments, interventions, documentation, medication administration, vaccines, and point-of-care testing in in-person, telephone, and telehealth settings. NH/compact LPN license and BLS required.

Qualifications

• Current LPN with NH or compact state license.
• Current BLS/CPR certification required.
• Graduate from an accredited nursing program or enrolled to obtain RN licensure.
• NH license as LPN required.

Responsibilities

• Perform nursing assessments and interventions under RN/provider supervision.
• Participate in care planning with interdisciplinary team.
• Educate patients on self-management, medications, nutrition, and wellness.
• Document nursing process accurately in medical records.
• Administer prescribed medications, vaccines, and perform point-of-care testing.
