Job summary

Benchmark Senior Living in Ridgefield, CT is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care in a home-like environment. You will assess residents, document changes, and supervise Resident Care Assistants while coordinating with care teams.

Requirements include an active LPN license and CPR certification; 1–2 years of nursing experience is preferred, especially with the elderly.

Qualifications

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program.
• 1–2 years nursing experience preferred.
• Previous experience working with the elderly is highly desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports changes in health to appropriate staff while per state regulations.
• Directs and supervises daily tasks of Resident Care Assistants.
• Mentors Lead Resident Care Assistants in job responsibilities.
• Coordinates care needs with community providers via case management.
• Balances service demands with resident independence.
• Recognizes and reports elder abuse or exploitation per state regulations.
• Responds to emergencies and activates backup as needed.
