Qualifications

• Valid Practical Nurse License (LPN) in the state where work is performed.
• 1+ years of experience in discharge planning or care coordination within a healthcare setting.
• 3+ years of experience in a Skilled Nursing Facility is required.
• Strong organizational and communication skills.
• Familiarity with medical terminology and charting practices.

Responsibilities

• Supervise and coordinate the discharge planning process with the healthcare team and families.
• Conduct comprehensive assessments of discharge needs including medications and equipment.
• Develop individualized discharge plans with residents and caregivers.
• Educate residents and families about care, medications, and follow-up care.
• Coordinate transportation and ensure timely discharge arrangements.
• Complete discharge paperwork and ensure all documentation is accurate and complete.
• Monitor residents during discharge and address any issues.
• Collaborate with the Director of Nursing to ensure regulatory compliance.
• Participate in team meetings to discuss progress and barriers to discharge.
• Perform additional duties as assigned by the Director of Nursing.
