Job summary

Luminis Health, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver direct patient care in a collaborative health team. The role emphasizes accurate assessment, care planning, and safe medication administration within established policies.

The LPN will work with Registered Nurses and other professionals to implement and adjust plans of care, participate in quality initiatives, and contribute to staff education and patient advocacy in a dynamic clinical setting.

Qualifications

• HS diploma/equivalent required.
• Two years of experience preferred.
• Active LPN licensure in Maryland.
• American Heart Association BLS required.
• Knowledge of nursing standards and policies.

Responsibilities

• Provide direct patient care to patients and families in a culturally appropriate manner.
• Assess patients regularly to collect data and identify learning needs per standards and policies.
• Collaborate with the care team to develop an individualized plan of care for each patient.
• Administer medications and manage pain following approved protocols.
• Organize and reprioritize patient care activities; serve as 1:1 sitter as needed.
• Evaluate the plan of care effectiveness and suggest revisions.
• Communicate changes in patient status to the health care team.
• Participate in quality improvement, CPI and risk management activities.
• Participate in unit governance and staff education/orientation.
