Luminis Health, Inc. is seeking a Licensed Practical Nurse (LPN) to join our care team in Maryland. The role emphasizes direct patient care, data collection, and collaborative planning with RNs to implement individualized patient plans.

The LPN will administer medications, manage pain, and participate in quality improvement activities while supporting education for new staff. Two years of experience is preferred, with a Maryland LPN license and BLS required.

Qualifications

• Directly provides patient care in a culturally appropriate manner.
• Thorough patient assessment to identify learning needs.
• Collaborates with care team to develop individual plans.
• Administers medications and manages pain as prescribed.
• Communicates changes in patient status to the team.

Responsabilités

• Provide direct care to patients and families in a culturally, developmentally, and appropriate manner.
• Consistently and thoroughly assess patients to collect data and identify learning needs.
• Collaborate with health care team to develop and individualize a plan of care.
• Implement the plan of care and demonstrate proficiency in medication administration and pain management.
• Organize and reprioritize patient care activities; act as a 1:1 sitter as needed.
• Evaluate effectiveness of the plan of care and revise as needed.
• Communicate changes in patient status to the health care team.
• Participate in QI, CPI, and risk management activities.
• Participate in unit shared governance and education of new staff.
