Job summary

Mercy Health in Ohio is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care and administer medications under RN supervision. You will participate in the nursing process, collaborate with a multidisciplinary team, and ensure accurate data collection and documentation.

The role requires an active LPN license, BLS certification, and 1 year of clinical care experience. EPIC EHR training and basic arrhythmia knowledge are preferred for telemetry units.

Qualifications

• Current LPN license from the state of employment.
• BLS Basic Life Support certification - American Heart Association (required).
• ACLS/PALS/NRP may be required for specialty areas.
• 1 year of clinical patient care experience in an acute care setting, long-term care facility, or physician practice (preferred).
• EPIC Electronic Health Record (EHR) training (preferred).
• Basic Arrhythmia class with passing grade (required for Telemetry units).

Responsibilities

• Under supervision of a Licensed Professional (e.g., Physician, PA, NP, or RN), participates in the nursing process to include data collection, planning, implementation, and evaluation of the patient’s plan of care.
• Gives direct nursing care and assists with care of the patient at the direction of a RN, Licensed Physician, PA, or APN.
• Contributes to the plan of care with input from patient/family and other healthcare team members.
• Performs patient data collection and observation considering age group and condition, coordinating resources and referrals.
• Collect and document objective and subjective data, and patient care including focused observations.
• Implements patient care in a safe, knowledgeable, and continuous manner.
• Administers ordered medications and treatments within scope of care.
• Performs phlebotomy services.
• Documents patient responses to care and communicates with the team.
• Recognizes and reports significant changes in the patient’s condition.
