Job summary

Southwest-Lt is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care in a skilled nursing facility under supervision of an RN or Director of Nursing. Responsibilities include administering medications, monitoring resident health, and aiding in the development of individualized care plans.

The role requires an active LPN/LVN license in the state, CPR certification, and experience in long-term care is preferred.

Qualifications

• Current LPN/LVN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or skilled nursing preferred.
• Current CPR certification.

Responsibilities

• Provide direct nursing care to residents per care plans.
• Administer medications and treatments as prescribed.
• Monitor residents’ vital signs and responses to treatment.
• Report changes to RN or physician.
• Assist with development and implementation of care plans.
• Supervise and support CNAs.
• Maintain medical documentation.
• Participate in shift reports and interdisciplinary meetings.
• Ensure infection control and safety standards.
• Respond to resident needs and emergencies.
