Qualifications

• Current registration with the Maryland State Board of Examiners of Nurses as a Licensed Practical Nurse.
• CPR Certification – not required for Hospice employee.

Responsibilities

• Assists in gathering data to assess patients' physical, psychological, social, and spiritual needs.
• Collaborates with the RN and team to plan for patient needs on admission and updates care plans.
• Provides direct patient care using nursing skills and documents outcomes in medical records.
• Administers and documents medications according to policy and procedure.
• Reports to RN with concise updates on patient condition changes.
