Job summary

Capital Health Plan's Center for Healthy Aging in Tallahassee, FL is seeking a dedicated Licensed Practical Nurse to join our nursing team. This full-time position offers direct patient care, coordination of healthcare services, and collaboration with physicians in a clinical setting.

The candidate will document in the EHR, follow the Florida Nurse Practice Act, and perform nursing orders under physician guidance while supporting organizational quality initiatives and patient-centered care.

Qualifications

• Certificate from a college or technical school.
• Current LPN license in Florida and Multistate Compact.
• BLS Certification.
• 2+ years of related experience (preferred).
• Ability to read and interpret safety rules, operating and maintenance instructions, and procedure manuals.
• Ability to write routine reports and correspondence.
• Ability to communicate effectively with patients, customers, and employees.
• Ability to perform mathematical calculations involving whole numbers, fractions, decimals, ratios, percentages, and graphs.
• Ability to apply common-sense understanding to carry out written, oral, and diagram-based instructions.
• Ability to work with computers, related technology, and software efficiently.
• Knowledge of medical safety procedures, infection control precautions, Nurse Practice Act standards, and Capital Health Plan policies and procedures.
• Ability to meet the physical demands of the position, including standing, walking, sitting, reaching, lifting, and moving patients or equipment.

Responsibilities

• Provide direct patient care and coordinate delivery of healthcare services.
• Prepare patients for examinations and treatment and document in EHR.
• Follow Florida Nurse Practice Act and work under physician direction.
• Carry out nursing orders in person, by phone, and via patient portal.
• Monitor patient responses to interventions and communicate outcomes to providers.
• Initiate specialty referrals as directed by physicians.
• Maintain clinical and medical supplies in a neat, orderly environment.
• Support Patient-Centered Medical Home and quality initiatives.
