Job summary

Capital Health Plan in Tallahassee, FL is hiring a full-time GPN - Certified LPN to support patient care across channels. You will coordinate with providers, document care, and ensure adherence to CHP protocols while contributing to positive patient outcomes.

The ideal candidate has 1 year of nursing experience (not required for the right person) and holds Florida LPN licensure or Graduate Practical Nurse status. Windows-based software proficiency is required.

Qualifications

• Must have Florida LPN license or Graduate Practical Nurse status as acknowledged by Board of Nursing.
• At least 1 year nursing experience preferred but not required for right candidate.
• Tech proficiency with Windows-based software and EHR workflows.

Responsibilities

• Coordinate with provider or RN to implement effective treatment plans.
• Follow CHP protocols and healthcare guidelines in all activities.
• Interact with patients via in person, phone, or electronic channels.
• Document clinical care in the system and support care gap closure.
• Help ensure timely, high-quality patient care delivery.
