Job summary

Southwest-Lt in Oklahoma City is seeking a Licensed Practical Nurse (LPN) to deliver direct nursing care in a skilled nursing facility under RN supervision. You will administer medications, monitor health, and support care plans in accordance with state regulations.

You will report changes to the RN or physician, supervise CNAs, maintain medical documentation, and participate in shift reports and team meetings.

Qualifications

• Must hold current LPN/LVN license in Oklahoma.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or skilled nursing preferred.
• Current CPR certification required.
• Strong communication and teamwork skills, plus solid documentation abilities.

Responsibilities

• Provide direct nursing care to residents per care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor residents' vital signs and responses to treatment.
• Report changes in condition to RN or physician.
• Assist with development and implementation of individualized care plans.
• Supervise and support CNAs.
• Maintain accurate medical documentation.
• Ensure infection control and safety standards are followed.
• Respond to resident needs and emergencies; participate in shift reports and team meetings.
