Job summary

CFG Health Systems, LLC is seeking a Licensed Practical Nurse to work closely with Registered Nurses and other health professionals. The role involves data collection, health assessments, and patient education as part of the nursing process.

You will record observations, identify goals, and support care planning while ensuring safety and privacy standards. The position requires adherence to facility policies and active state LPN licensure.

Qualifications

• Graduate of an accredited Licensed Practical Nurse program with current licensure in the state of employment.
• Must conform to the security clearance standards of the institution where employed.

Responsibilities

• In a team relationship with the RN supervisor, contributes to nursing assessment, care and planning.
• Initiates the admission process and assists with the identification of recurrent health problems, symptoms and behavioral changes.
• Collects pertinent data through observation, interview, and other members of the health team and communicates changes in the patient condition to other patient care providers.
• Records all data obtained through assessment of the patient’s health record and assists in forming a written plan.
• Assists in identifying short and long term goals with anticipated behavioral change.
• Assists with prioritizing care to meet patient needs.
• Assists with patient health care and participates in multi-disciplinary care conferences.
• Assists with administration of routine medications and transcribes prescribed orders.
