Shorewood Senior Living in Rochester, MN is seeking a Licensed Practical Nurse (LPN) to join a Silvercrest Community. You will administer medications, assess residents, and develop care plans while guiding unlicensed staff and supporting residents with dementia with compassion.

Qualifications

• Current, active Minnesota LPN license.
• Two or more years of experience in assisted living, skilled nursing, or home care is preferred.
• Experience with geriatric assessment, dementia care, and supervising unlicensed staff is a plus.

Responsibilities

• Administer medications and treatments according to physician orders and care plans.
• Complete resident assessments and monitor for changes in condition, communicating with providers, families, and the care team.
• Implement and update resident-centered care plans.
• Document care accurately and on time in resident records.
• Direct and support unlicensed staff, such as caregivers and medication aides.
• Support residents living with Alzheimer’s and other forms of dementia with patience and dignity.
• Deliver excellent customer service to residents and their families in every interaction.
