Job summary

REM Community Services, a part of the Sevita family, is seeking a Licensed Practical Nurse to join a mission-driven team and provide nursing services and supports to individuals with complex medical or behavioral needs.

This daytime, Monday–Friday role in Minnesota offers meaningful work with a focus on health monitoring, medication administration, and staff training. A nursing degree and LPN license are required.

Qualifications

• Associate's Degree in Nursing or related field.
• One year of nursing experience with the population served.
• Current state LPN or LVN license.
• Valid driver's license, car registration and auto insurance.
• CPR/FA, HIV/AIDS, BBP and ACT training as required by state regulations.

Responsibilities

• Monitor the health conditions of individuals served and ensure quality services are provided.
• Administer medications and provide documentation as well as oversee and monitor staff members who administer medications.
• Provide prescribed medical treatment by physician’s orders and personal care services.
• Conduct assessments, implement health care services to be included in each ISP, and respond to changes of condition.
• Provide training to staff members including universal precautions, infection control, and medication administration.
• Serve as liaison and advocate with other medical and professional service providers.
• Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention and smoking cessation.
