Job summary

Heritage Wellness & Rehabilitation is seeking a dedicated Licensed Practical Nurse (LPN) for a rewarding day shift in a compassionate, resident-centered environment. Provide high-quality care, monitor conditions, administer medications, and collaborate with an interdisciplinary team to ensure safety and comfort.

Qualifications

• Current Missouri LPN license in good standing.
• BLS/CPR Certification.
• Experience in skilled nursing, rehabilitation, long-term care, hospital, or post-acute care preferred.

Responsibilities

• Provide skilled nursing care according to physician orders and individualized care plans.
• Monitor residents and identify changes in condition.
• Administer medications and treatments as ordered.
• Provide wound care and other nursing treatments within scope of practice.
• Communicate changes in resident condition to the RN, physician, and appropriate members of the care team.
• Respond promptly to resident needs, call lights, and emergencies.
• Complete accurate and timely nursing documentation.
• Assist with admissions, transfers, and discharges as needed.
• Collaborate with RNs, CNAs, physicians, and the interdisciplinary team.
• Provide compassionate care while promoting resident dignity, safety, and comfort.
• Follow infection prevention and infection control protocols.
• Participate in QAPI initiatives.
• Maintain compliance with CMS, state, and facility regulations.
