Job summary

Heritage Wellness & Rehabilitation is hiring a dedicated LPN for the day shift to deliver compassionate, resident-centered nursing care in a skilled nursing and rehabilitation setting. You will work with an interdisciplinary team to provide safe, high-quality care per physician orders and care plans.

Ideal candidates hold a Missouri LPN license and BLS/CPR, with strong clinical skills and a focus on teamwork. This full-time role includes a $3,000 sign-on bonus and comprehensive benefits.

Qualifications

Current Missouri LPN license in good standing.
BLS/CPR certification is required.
Experience in skilled nursing or rehabilitation settings preferred.
Excellent communication and teamwork abilities.

Responsibilities

Provide skilled nursing care according to physician orders and care plans.
Monitor residents and identify changes in condition.
Administer medications and treatments as ordered.
Document accurately and timely nursing notes.
Collaborate with RNs, CNAs, physicians, and the care team.
Support admissions, transfers, and discharges as needed.
Follow infection control protocols and CMS/state regulations.
Contribute to QAPI initiatives and maintain resident dignity and safety.
