Job summary

Heritage Wellness & Rehabilitation is seeking a Licensed Practical Nurse (LPN) for the night shift to provide compassionate, resident-centered care in a skilled nursing and rehabilitation environment.

You will monitor residents, administer medications, coordinate with the care team, and participate in QA and infection control while upholding CMS and facility policies. A $3,000 sign-on bonus is offered for full-time night shift nurses.

Qualifications

• Current Missouri LPN license in good standing.
• BLS/CPR certification.
• Experience in skilled nursing or similar setting preferred.
• Strong clinical and organizational skills.
• Excellent communication and teamwork.

Responsibilities

• Provide skilled nursing care according to physician orders and individualized care plans.
• Monitor residents and identify changes in condition.
• Administer medications and treatments as ordered.
• Provide wound care and other nursing treatments within scope of practice.
• Communicate changes in resident condition to the RN, physician, and appropriate members of the care team.
• Respond promptly to resident needs, call lights, and emergencies.
• Complete accurate and timely nursing documentation.
• Assist with admissions, transfers, and discharges as needed.
• Collaborate with RNs, CNAs, physicians, and the interdisciplinary team.
• Provide compassionate care while promoting resident dignity, safety, and comfort.
• Follow infection prevention and infection control protocols.
• Participate in QAPI initiatives.
• Maintain compliance with CMS, state, and facility regulations.

Position Summary

The Licensed Practical Nurse (LPN) provides compassionate, high-quality nursing care to residents in accordance with physician orders and individualized care plans. The LPN monitors resident conditions, administers medications and treatments, communicates changes in condition to the appropriate members of the clinical team, and works collaboratively with RNs, CNAs, physicians, and other healthcare professionals.

Qualifications

• Current Missouri Licensed Practical Nurse (LPN) license in good standing

• Current BLS/CPR Certification

• Skilled nursing, rehabilitation, long-term care, hospital, or post-acute care experience preferred

• Excellent communication and teamwork abilities

• Compassionate, dependable, and resident-focused

• Ability to work independently and effectively on the night shift
