Qualifications
- Minimum one year experience with individuals with disabilities or elderly population.
- Ability to assist with activities of daily living and transfers.
- Willingness to obtain CPR and first aid certification if not already certified.
Responsibilities
- Perform health monitoring for program participants.
- Coordinate participant admissions and discharges.
- Document participant health records and report concerning trends.
- Provide education on health and wellness to clients and families.
- Support program staff with clinical training and knowledge.
- Schedule medical appointments and coordinate transportation for participants.
