Job summary

Easterseals Florida is seeking a compassionate and dynamic Substitute Nurse (LPN) to join our Winter Park adult day health care center. This position involves performing health monitoring and nursing services for adults with disabilities or dementia-related disorders.

Qualifications

• Graduate of an accredited nursing program.
• Current Florida Licensed Practical Nurse License required.
• Experience with disabilities, special needs, and the aging population preferred.

Responsibilities

• Coordinate participant admissions and discharges and manage medical records.
• Schedule medical appointments and transportation for participants.
• Monitor health progress and report concerning trends to the Center Director.
• Provide education to participants and caregivers on health and emergency resources.

$28.00 per hour, As-Needed Position

Monday - Friday ONLY, No Nights, No Weekends, No Holidays!

Easterseals Florida is seeking a compassionate, professional and dynamic Substitute Nurse (LPN) to join our team in our Winter Park adult day health care center, which serves adults with disabilities, dementia related disorders or other memory impairment conditions. You will perform all health monitoring and nursing services for participants in the program. The Substitute Nurse provides excellent care and customer service in a person-centered setting designed to enhance the quality of life for our participants.

Responsibilities

• Assist in coordinating admissions and discharges, building new participant charts/medical records, ensuring admissions paperwork is completed.
• Assist in scheduling in-house medical specialty visits, medical/health appointments, and transportation as needed.
• Track, review, and coordinate quarterly care plan meetings with participants and family/caregiver team.
• Take, document, and maintain records for each participant, reporting trends outside of parameters, or needing attention from the Center Director.
• Provide clinical support, insight, knowledge, and applicable in-service training to program staff.
• Provide education to participants and family/caregiver team on health and wellness, medical diagnosis, medication, safe living, emergency situations, and resources available.

Qualifications

• Graduate of an accredited nursing program
• Current Florida Licensed Practical Nurse License
• At least one year of experience working with individuals with disabilities and special needs and/or the aging population and Alzheimer's patients
• Ability to assist with the transfer of participants to and from chair or wheelchair and assist clients with activities of daily living (toileting, bathing, ambulating, feeding, changing clothes, etc)
• CPR and first aid certification a plus, but can be obtained/renewed once hired
